For New Clients:
Before your first appointment with me I will ask you to review and fill out some paperwork. You may download the appropriate form below or, if you prefer, you may contact me and I will mail the forms to you. Please select from the following:

The forms are Adobe PDFs and you may download the free reader from Adobe to open the documents. I will require the completed forms at our first visit or, preferably, mailed before our first visit to my office at 2732 NE Broadway, Portland, OR 97232.
I treat the information you provide -- in writing, in person, and in voice - and email messages -- as strictly confidential. But please recall that email is not a secure method of communication, so I encourage you to be circumspect in what you send to me though that medium.
For All Clients:
My regular office hours are Monday through Friday daytime hours by appointment. I will also schedule Saturday morning hours and some weekday evening hours by appointment. I will make every effort to accommodate your schedule. My evening and weekend hours are limited and so tend to fill up very quickly; I tend to have greater availability and flexibility during weekday hours.
Please be aware that the time we schedule for your therapy is time I have reserved for you. If at any time you find you will be unable to attend a scheduled appointment please contact me by phone at 503-421-1597 as soon as possible so I might make that time available to someone else. I will charge a $50 fee for missed appointments.
Therapy Dog:
Currently I work one or two days a week with a therapy dog named Lucy. She is a golden retriever-poodle cross (the unfortunately-named
"Goldendoodle") and is an AKC Canine Good Citizen. Many people find the presence of a dog to be comforting or relaxing; however, many others find the work of therapy goes better without the distraction of a dog. I respect both positions and will be glad to accommodate your preference.
